Information we collect

Information you give us

When you schedule a visit, call, text, or email us, you may give us:

  • Your name, phone number, email address, mailing address, date of birth, and preferred language
  • Details about your accident, your injuries, and your symptoms
  • The name of your attorney or law firm and information about your claim
  • Documents you send us, such as records, photos, or reports

Information from others

  • Your attorney or law firm, when they refer you to us
  • Licensed providers in the Ascend Care network who treat you, such as visit notes, records, and bills

Information collected automatically

When you visit this website, our server records basic technical details. See This website.

How we use your information

  • To schedule your visits and coordinate your care with licensed providers
  • To contact you by phone, text message, or email about your care
  • To send records and bills to your attorney under your Letter of Protection
  • To answer your questions and give you support
  • To run, secure, and improve our services
  • To meet our legal, regulatory, and record keeping duties

We do not sell your personal information. We do not use it for advertising.

Health information

Health information connected to your care is protected health information under HIPAA, the federal health privacy law. We use and share it only as HIPAA allows: to treat you, to bill and get paid for your care, to run our operations, and as you authorize. For example, we share records with your attorney when you have authorized it.

Under HIPAA you have the right to see and get a copy of your health records, ask us to correct them, ask for a list of certain disclosures, and file a complaint. You can ask us for a copy of the Notice of Privacy Practices that applies to your care at any time.

Text messages (SMS)

Ascend Care sends text messages about scheduling, appointment reminders, visit follow up, and documents we need for your care. You will only get texts from us if you gave us your mobile number and agreed to receive them. Agreeing to texts is never a condition of receiving care.

Stop or get help
Reply STOP to opt out. Reply HELP for help.

When you reply STOP, we will send one final message to confirm you are opted out, and you will get no more texts unless you sign up again. For help, reply HELP, call us at (832) 278-1677, or email monica@myascendcare.com. Wireless carriers are not liable for delayed or undelivered messages.

We try to keep medical details out of text messages. If we need to share health information, we will use a more secure method or call you.

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes.

Information sharing with subcontractors in support services, such as the company that delivers our text messages, is permitted. All other categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

Phone calls

Calls with Ascend Care may be recorded and transcribed so we keep an accurate record of your care and improve our service. We will tell you when a call is recorded.

Some calls may use interpretation, transcription, or automated voice technology, for example to help patients who prefer to speak Spanish. Ascend Care staff oversee these calls.

How we share information

We share information only as needed to provide and pay for your care, or when the law requires it:

  • Licensed providers in the Ascend Care network who treat you
  • Your attorney or law firm, as part of your Letter of Protection and as you authorize
  • Service providers who help us operate, such as phone and text message delivery, email, secure hosting, and billing. They may use your information only to do work for us. Those that handle health information sign agreements that require them to protect it, as HIPAA requires.
  • Government agencies, courts, or others when the law requires it, such as for a subpoena or court order, or to protect someone's health or safety

We never sell your information or share it with others for their marketing.

This website

This website does not use advertising cookies or third party tracking tools. Like most websites, our server keeps standard logs, such as your IP address, browser type, the pages you visit, and the time of your visit. We use these logs to keep the site working and secure.

Our pages load fonts from Google Fonts, so Google may receive your IP address when your browser requests them. The staff portal linked from this site uses a sign in cookie for Ascend Care staff only.

Security and retention

We protect your information with safeguards such as encrypted connections, access limited to staff who need it, strong sign in controls, and activity logs. No system is perfectly secure, but we work to protect your information and will notify you if the law requires it after a breach.

We keep information as long as we need it to provide services, meet medical record and legal requirements, and resolve disputes. Then we delete it or keep it in a form that no longer identifies you.

Your choices

  • Text messages: reply STOP at any time to stop texts.
  • Email: ask us to stop sending email that is not needed for your care.
  • Your records: ask to see, correct, or delete information we hold about you. Some records we must keep by law, and we will tell you if that applies.

This website is not meant for children under 13. We collect information about a minor only as part of their care, with a parent or guardian involved.
